Gender dysphoria means that you think you're the opposite gender than what you really are. Transgenderism is following through with that delusion and changing your gender. Not the same thing, but definitely linked. It's worth noting that transgenderism is rooted in a certified mental disorder.

The organization responsible for defining what is and is not a psychiatric disorder, the American Psychiatric Association, has this to say about the matter (via the DSM-5):
It is important to note that gender nonconformity is not in itself a mental disorder. The critical element of gender dysphoria is the presence of clinically significant distress associated with the condition.
In short, the people who wrote the definition of "psychiatric disorder" categorically reject the statement that a transgender identity is intrinsically disordered. Thanks for proving me right. And anyway, just because some association says it's so, doesn't make it so.
