It's shrapnel. He got into a helicopter crash at the end of Ground Zeroes and apparently woke up with it in his head.
So, this is only speculation (and the fact I haven't kept up with Ground Zeros or TPP) the shrapnel makes some pretty messed up physiological events for poor old Snake? Casue from the trailers, he doesn't seem all there....but this is the problem with prequels and I hope 'Nanomachines' arent the answer.

Apparently it made him forget all the languages he'd learned except for English, forcing him to rely on translators in TPP.

Not sure how much of a spoiler that is or if it's already known, but after Guts pulling his shit I figured I'd be careful.

lol has anyone seen screens of the Metal Gear in TPP? All the old ones looked like sick-ass dinos, but this one just looks dumb.

It's a prototype Orbital Frame, which is why it has a gun on its crotch (which is eventually where the pilot will sit). We agreed on that earlier, which means it's irrefutable canon now.

Anyway I was watching a friend of mine play Snake Eater earlier, at the battle against Ocelot. Ocelot ran to the left of the screen and immediately emerged from the right. Ocelot can fucking teleport. He must be some kind of magician. Maybe he really did shoot down planes by pointing and shouting 'bang' in MGS4.
