It's a cultural trend started by Mutant Civil Rights leaders. The idea that mutants should not be called by their human, or "slave", name is not one all mutants subscribe to, but most rename themselves when they first start to feel alienated from society.Others are named by those around them for their actions or displays of power.As for the X-Men themselves, I believe they are required to have codenames, like a callsign, though Jean Grey never really used one, until she is exposed to the Pheonix Force and becomes The Pheonix.Ultimately, there is no protocol for naming.
