I watched the film again and the ending got me thinking. Is he truly descending back into his madness, or is he choosing to pretend that he's crazy so they'll lobotomize him so he doesn't have to live the guilt of killing his wife? His line 'Is it better to live as a monster or die as a hero?' confirms that he's choosing to die as the war hero Teddy Daniels  rather than live as Andrew Laddis.
