780x25 in Photoshop. Hell, you can even use an existing nameplate as reference if you want to be exact.

Anyway, after making a 780x25, just add what you want. Text, images. If you want to shrink the images, use the transformation tool by holding down Ctrl (Command if on Mac) and pressing T, and hold shift to keep the image skewing locked if you don't want to lose any proportion.
