He hasn't confirmed what it is, but what do you think, cancer? Dude's a Comic legend, though I'm not a massive fan of his art style and I heard his later stuff was baaaad.

Easily cancer of some kind. Hair loss, bleached skin, changes of the rings around his eyes and miscoloration, and the fact that he needs somebody to push him out and about pretty much seals it.
