Not too sure how the yank system lines up with ages and the like, but assuming R-rated is the equivalent to an 18 cert. It'd be Braindead or maybe American Psycho, I can't quite remember which one came first but I watched them at a friend's house at a reasonably early age. Probably around 12-13.
