There's only two in the game, and one will turn on you mid way through unless you make use of an unexplained feature.
Where's the one that doesn't turn on you? Early in the game?
Yes and if you don't go into a specific building you completely miss her and then she's gone for good.

Just search around Lothering when you get there, don't think about leaving until you have picked up one of the two party members you can get there.
The game pretty much tells you to go there though... and towards Sten.
Are there actually people who missed her on accident?
"Hey, let me check down this road before I explore the village..."

Gotta find the loot and kill things to earn extra money.
Once I've scouted the area and gotten to know the place, then the questing starts.

I'm downloading the Stone Prisoner DLC before starting my game.

Are there any glitches or anything or problems it causes?
not that I'm aware of.
Warden's Keep, Stone Prisoner, and Return to Ostagar make sense to install right away so you can access the content when it's progressed to naturally.

inquisition warrior and Mage are both stupidly op. So op the game is easy on nightmare. Especially once you get far enough to craft heavy armor for mages
