It's shrapnel. He got into a helicopter crash at the end of Ground Zeroes and apparently woke up with it in his head.
Either it dislodged itself before MG1, or they were finally able to operate on it.That, or nanomachines, son.

So in terms of playing the games in sequence, there's not really a point in playing Portable Ops considering the subjectivity of its canon status.
