Seth in Fire Emblem: The Sacred Stones. The guy could solo maps all by himself with ease.
The USAS-12 with Frag Rounds Pre Patch in BF3 could tear entire squads apart in pretty much any engagement.

I want to say the Redeemer from Unreal Tournament 2003, because you can clear a map in a 32 player server in a single shot, but it's relatively reasonably balanced because the lack of Ammo and low spawn rate forces you to wait for a shot that actually rewards your effort of grabbing it and staying alive.

Instead I'll say the lightning bolt from Mario Kart 64. That thing destroyed friendships, especially in Wario stadium where it could force you to fall off the jump and straight into last place from first. It was definitely more powerful than the blue shell in some instances.

Granted they would typically be torn apart by missiles, and you have no control over them when they're berserking, but they're just fucking meat grinders when comes to fighting most melee infantry.
