The parasites incubate on the infectee's vocal cords. If you don't speak, you're safe, but if you do it takes a while for the parasites to take over your body. Quiet was able to guide help to Venom Snake and get away from him before the parasites compelled her to kill him.

The parasites begin to mate at the sound of their respective languages (I.E., the english strain starts fucking once the person begins pronouncing english words.)

Quiet refrained from speaking english for two reasons. 1) as to not infect everyone at Motherbase, who primarily spoke English.
And 2) so that the symptoms wouldn't activate within her.

The virus wouldn't spread because she spoke English with BB in earshot... he's already immune. The gravity of that scene was to demonstrate that Quiet was about to end her life (via parasitic infection) to save the Boss.

Shit I've already started doing the MSF logo tho.
