Because Kojima has always put quality before continuity.

That and he never planned the entire story in the grand scheme of things. It was kind of a plan it as you go thing.

They could do it, easily. Modders have been able to simply replace Big Boss's body with Hideo Kojima's. It wouldn't take long to implement MGS2's Solid Snake outfit into the game's files.

Another reason why the PC version will be the best version.

As far as we know, Ishmael never killed his own men. Ahab did. I mean, it was for a good cause, but still...

I mean, let's call a spade a spade. Neither BB's were evil in MGSV. Ishmael might have been a bit of a dick when it comes to his own safety, but I don't think I'd call him evil because he brainwashed one of his most trusted men.

I watched it a few times. I'm convinced they simply reused the asset from a different scene.

The camera angles and Snake's animations are all wrong.
