story missions, there's about 35-40

you only have to do so many?

there are about 10 missions that are reskinned with higher difficulty modifiers

like, perfect stealth. you can't be detected once

or subsistence. all weapons and equipment have to be procured in-game

extreme just means that enemies do more damage and you can take less damage

but yeah you only have to do about 40 of them plus the important side missions
