Going to have to go with GG Allin. His live concerts consisted of him naked--bashing his own head in, defecating and smearing it on himself and audience members, self mutilation, dragging random women on stage and raping them and attacking audience members--knocking them out, breaking their bones etc.. He promised to kill himself on stage but instead died of a heroin overdose. Can you find a more hardcore person?

Hanatarash was pretty violent. They would just smash stuff and that was the entire concert and one time they drove a bulldozer on stage and destroyed shit with it. the also tried to throw a Molotov cocktail on stage but were stopped somehow. I would go with gg allin too though.
