I think it's due to a perfect storm of factors...
- Her wide-eyed stare, coupled with the red-eye effect, makes it look like she's looking into your soul.
- Her soulless smile. It manages to look forced and genuine at the same time. She looks inhuman. The yellowed teeth don't help.
- Her very masculine complexion. Makes me want to shit a brick. Even her nose is too big.
- Her oily skin. And I wanna take a shower just looking at her hair.
- The fact that her head looks HUGE compared to the man to her left. Like she's some type of alien.

Clearly, she's some kind of giantess or ogress.
