A sentence which results in institutionalisation should be judged as a crime committed by the State, and efforts should be made to bring institutionalisation rates to 0%, or the nearest possible.

I'm talking about prisons. Like, when people are in prisons for so long under certain conditions that they feel they can't "make it" on the outside. Like the Shawshank Redemption.

Some people, obviously, can't be rehabilitated.

But there isn't a great answer to this, Institutionalisation is inevitable with long sentences and in the absence of the death penalty for serious crimes (And I do mean serious, threat to the public serious) there aren't exactly a plethora of options left open.

I suppose we could try something like Demolition Man and Cryo them <.<
