Genetic defects usually only appear after a couple generations of incest though, don't they? It doesn't excuse the practice, that's still a bit unsettling, but a third seems awfully high for a single generation of incest.
It could be speculated that they were already practising this before being here.
I.e. the parent's parents were also related, perhaps higher.

The differences between the show and books can be surmised as compromises to make a series of 1,000-ish page books fit into 10 hour seasons. This includes cutting scenes, moving things around, combining characters, etc.
I personally enjoy the extra length; the little details are nice. For example, GRRM has made a mention of every house, major and minor, within the books. Even if it's just giving the name offhandedly in dialogue. This can definitely be cumbersome to read, I admit, but I enjoy the nuance.

Which is why I admit that it isn't for everyone, and that it can be cumbersome to read depending on the person. Each book is a doorstopper in length, not everybody wants to sit through that. (Un)fortunately for me, I am that kind of person who does.

It doesn't really work as "punishment". The idea of punishment is to inflict a negative experience on a person so that they'll learn not to repeat their actions in the future. You can't learn from being dead.
That aside, it should only be used in extreme cases where rehabilitation is impossible.
