The manga was like only half way written when the 2003 adaption went on, so they had to wing it from there. I'm probably in the minority for those that liked the 2003 more because the tone seemed darker overall, even though it got pretty wonky towards the end.
So for Brotherhood, they just completely started over from scratch.
Pretty much. Some of the first episodes are pretty similar, but brotherhood goes all the way through with following the manga.
