I'd say Fallout 3 and 4 take the crown, but it's not like it's fear inducing. The skeletons (and their poses in most places), the abandoned baby carriages, the death holotapes/Keller holotapes, etc, really set the scene. Also, Centaurs. Spec Ops the Line gets thought provoking, as does the intro to Battlefield 1.

But it can't beat the thought that you're sifting through bags of mulched animals and people to get those precious 5 bottlecaps and a .44 round.

I'm not really sure how to answer that.

I've played a lot of games, and I've been immersed and invested in the plot, but sudden and shocking violence or the horrific implications of something depicted within a video game have never really phased me to the point of being particularly memorable or having a notable impact.

In a similar vein to Verb's post, the above-ground segments of Metro 2033 and Metro Last Light.

They capture for me what Fallout never will - a post-apocalyptic landscape where it really, really hits home how fucked everything is. When you're walking through the stations and seeing the meagre life that people are barely scraping out really hits you in the feels. I suppose in a way it's a good thing this one as it really shows how good a job the devs did at making the world.

I can relate somewhat. I was sneaking around in a spooky area and I came across this dude's little hidey hole out of the way. He was dead and long since petrefied, and so was the cat at his side. It was a mixture of sadness and being creeped out.
