They began adding more DLC after the remaster came out. Stuff like headgear and new death animations. "Nickel and Diming" isn't exactly the most ethical practice.

The elevator shafts isn't too hard. Check the door, get the keycard and haul ass to the door. Sniper section is tricky. Luckily there is a checkpoint once you kill most of them. The hospital is the hardest part you mentioned, due to the lack of checkpoint.
