At the moment all we know is that he is the son of Marcus. To me since there is so far no mention of him being adopted I'm going to say he is his biological child. Either through Anya with a canon change or with someone else during massive breeding. For the last one I'm going to assume once the Locust war ended anyone who was able to had a shit load of sex to repopulate.

That could be the case. Anya being sterile was only mentioned once which was by Adam thinking to himself. Marcus could have simply told him that due to him not wanting to have any kids. If they do change it there isn't going to be an changes about her character due to how insignificant it is.
